The Mapleson D adult Anesthesia Breathing Circuit is A Semi-Open Breathing System That is Most Efficient During Controlled Ventilation (IPPV)

 

Key Components:-

  • Fresh Gas Inlet: Located right next to the patient connection.

  • Corrugated Tubing: Connects the patient end to the reservoir bag and valve.

  • Reservoir Bag: Placed at the opposite end of the patient, near the expiratory valve.

  • APL Valve (Adjustable Pressure Limiting): Located at the machine/bag end, away from the patient to vent waste gases.

  • Coaxial Modification (Bain Circuit): An inner tube delivers fresh gas to the patient down the center of an outer corrugated expiratory tube.

 

Functional Characteristics

  • Controlled Ventilation: Highly efficient because incoming fresh gas pushes alveolar waste gas directly out through the APL valve. Fresh gas flow (FGF) requirement equals about 1 times the minute ventilation. 

  • Spontaneous Ventilation: Inefficient. The patient tends to re-breathe exhaled carbon dioxide unless high fresh gas flows are maintained. [1]
